If it was pitched to each club captain/organiser as a route to getting recognition and funding (or made a mandatory part of joining the BUF) then surely every club would reply. I think a form sent by each club would be the most viable option. This could be done on a single form, or preferably an e-mail questionaire, (surely every team must have some form of access to e-mail). I know it would be a hassle for one individual in each club (I'd have to do the UTI one) but if the benefits were sold in well it could work.
